Documents/details required to file Drug License application
- Passport size photocopy of applicant and pharmacist.
- Declaration by the Proprietor / Partner / Director / Competent Person / Registered Pharmacist with proof of residential address.
- Proof of residential address such as Aadhaar Card, Passport, Driving License or Voter ID.
- Partnership deed in case of partnership firm.
- List of Directors downloaded from MCA website and signed by Company Secretary / Managing Director, in case of company.
- Affidavit under section 34 of the Drugs and Cosmetics Act, 1940 on ₹20 stamp paper signed by one of the Directors of the company, in case of company.
- Declaration of the Registered Pharmacist regarding acceptance of job on ₹20 Non-Judicial stamp paper in prescribed proforma.
- Declaration of the Proprietor regarding full time employment of Pharmacist on ₹20 Non-Judicial stamp paper in prescribed proforma.
- Declaration of the Proprietor cum Pharmacist on ₹20 Non-Judicial stamp paper in prescribed proforma duly signed by public notary, if the pharmacist himself is a proprietor.
- Declaration of the Proprietor regarding non-conviction under Drugs and Cosmetics Act and Rules on ₹20 Non-Judicial stamp paper in prescribed proforma.
- Declaration of building owner regarding no objection for conducting the business of retail drugs on ₹20 Non-Judicial stamp paper in prescribed proforma.
- Self-attested copy of Registered Pharmacist certificate, renewal up to date, affixed with latest original photograph and signature of the candidate.
- Self-attested copy of qualification certificate of the Registered Pharmacist.
- Copy of rent / lease deed in case of rental premises.
- Invoice of newly purchased refrigerator in the name of shop / proprietor.
- Sketch map showing the location in the premises.
- Photocopy of the premises run by the applicant.

Application fee is payable online through Bharatkosh after submission of application, photos and documents.
